Paycheck Protection Program (PPP) Applications

We are currently accepting First and Second Draw PPP loan applications.

Please note that during this phase, the SBA will not be granting on-the-spot approvals with new loan numbers as they had done during the previous PPP program. The SBA has stated that their internal review and approval with new loan numbers will be processed in an expedited manner, but it will take additional time. We appreciate your patience during this process and thank you for choosing First Florida Integrity Bank for your PPP loan.

 

If you have not previously taken out a PPP loan, please complete :

First Draw

1. The First Draw PPP Application
2. Customer information forms (Located under the New Customer Only links below)
3. Payroll tax reports (2019 or 2020 941 forms if a corporation/partnership, 2019 or 2020 Schedule C and 1099-MISC if a sole proprietor/independent contractor)
4. Payroll report by employee for the chosen year
5. Documentation to verify borrower had employees as of 2/15/2020 (e.g. payroll report that includes that date)
6. Schedule K-1 statements for partnerships
7. Documentation of employer paid insurance and retirement plans (if including in payroll costs for the year)

 

If you have previously received a PPP loan, utilized all of the funds and have experienced a 25% or greater loss in revenues in a calendar quarter in 2020 as compared to the corresponding quarter in 2019, please complete:

Second Draw

1. The Second Draw PPP Application
2. Payroll tax reports (2019 or 2020 941 forms if a corporation/partnership, 2019 or 2020 Schedule C and 1099-MISC if a sole proprietor/independent contractor)
3. Payroll report by employee for the chosen year
4. Documentation to verify borrower had employees as of 2/15/2020 (e.g. payroll report that includes that date)
5. Schedule K-1 statements for partnerships
6. Documentation of employer paid insurance and retirement plans (if including in payroll costs for the year)
7. If your loan is over $150,000, comparative financial statements or bank statements for the calendar quarters where you experienced the 25% or greater loss in revenues. ex. Q2 2020 and Q2 2019.
